Referring to the TBI Clinic
Referral Requirements
All new patients require a referral from a provider (primary care doctor, functional medicine provider, emergency department, or other care provider).
Referrals for TBI Clinic should include:
- Current patient contact information (updated address, phone, and/or family contact information)
- Date of brain injury (or approximate date)
Fax TBI Clinic referrals to our central referral center: 612-873-1635.
- Our TBI Clinic nurse reviews each referral to match patients with appropriate providers. Review process may take up to one week after receipt. The patient will be contacted to schedule once triage is complete.
Sending Supplementary Medical Records
- If your practice is not on Epic Care-Everywhere, please fax supplementary TBI-related medical records directly to our TBI Clinic Nurse: 612-873-8437
